Updated forms under Childrens Court Act 1992

As a result of the Justice & Other Legislation Act 2023 amendments to the Oaths Act 1867, the following updated forms have been approved for use from 28 July 2025 under the Childrens Court Act 1992:

• Form 25 - Affidavit (DOCX, 41.5 KB)

Youth Justice Act 1992

• Form 34 - Application and affidavit to vary a community based order (other than a conditional release order) by consent (DOCX, 29.4 KB)
