Application for probate
You will need to provide the following documents for an application for probate.

Affidavit in support: Form 100 – Probate title (UCPR) + Form 105** - Affidavit (UCPR)
Exhibits: original will and original death certificate + exhibit clause on Form 47 – Certificate of exhibit (UCPR)
Affidavit of publication and service: Form 100 – Probate title (UCPR) + Form 104** - Affidavit (UCPR)
Exhibit: Form 103 - Notice of intention to apply for grant (UCPR) + exhibit clause on Form 47 – Certificate of exhibit (UCPR).
Letters of administration
You will need to provide the following documents for an application for letters of administration.

Letters of administration with the will
You will need to provide the following documents for an application for letters of administration with the will.

Reseals
You will need to provide the following documents for an application for reseals (under the British Probates Act 1898).
Application: Form 100 – Probate title (UCPR) + Form 112** - Application of reseal (UCPR)
Affidavit in support: Form 100 – Probate title (UCPR) + Form 113** - Affidavit
Exhibits: if not the grantee, power of attorney may be required + exhibit clause on Form 47 – Certificate of exhibit (UCPR).
