As a result of the Justice & Other Legislation Act 2023 amendments to the Oaths Act 1867, the following updated forms have been approved for use from 12 May 2025 under the Uniform Civil Procedure Rules 1999 and the Criminal Practice Rules 1999:

Two court forms have been removed:
