Updated form under Uniform Civil Procedure Rules 1999
The new version Form 109 - Affidavit (letters of administration on intestacy) has been approved for use from 12 September 2025 under the Uniform Civil Procedure Rules 1999.
The form has been published on the Queensland Courts website on 16 September 2025 and is available on the Forms page.
