New and revised DFV Forms

New and revised forms have been issued to reflect the legislative amendments arising from the Domestic and Family Violence Protection (Combatting Coercive Control) and Other Legislation Amendment Act 2023, which commenced on 01 August 2023. The following forms have been developed or amended:

• DV01A Guide to Completing an Application for a Protection Order
• DV38 Application for Substituted Service Order (NEW)
• DV39 Affidavit of Attempted Personal Service (NEW)
• DV40 Substituted Service Order (NEW)
• DV41 Respondent’s Application to Reopen Proceedings After Service Under a Substituted Service Order (NEW)
• DV45 Statement of Substituted Police Service (NEW)
